How does the corneal flap affect visual outcomes?

How does the corneal flap affect visual outcomes?

A: New science reveals that the corneal flap has been widely underestimated for its affect on the visual outcome of LASIK. If the corneal surface is left with fine aberrations after the cut with the handheld blade, the precision of the excimer tissue ablation, the second step, can be compromised. Eliminating tiny irregularities on the corneal surface, as is possible with Bladeless LASIK, gives a boost to the vision after the procedure.

Surgeons have found statistically and clinically significant differences in the vision patients achieve when the femtosecond laser is used for corneal flap creation. Bladeless LASIK goes beyond simply creating a safer, planar flap to preparing an optimal corneal architecture below the flap, providing for superior visual outcomes, even among patients who undergo wavefront guided procedures. The end result: fewer complications and better vision.

Why is LASIK a double step procedure?

When laser vision correction was performed without the corneal flap, as in PRK (photorefractive keratotomy), patients experienced considerable discomfort and a delay in visual acuity. So in LASIK, the surgeon first creates a micro-thin flap of corneal tissue and folds it back to prepare the eye for the second step, where an excimer laser is used on the inner cornea to correct vision. This two-step process allows for rapid visual recovery with little or no patient discomfort, and is credited with the popularity and growth of the procedure worldwide.

The differences between different LASIK procedures is on the first step of this two step procedure-how the flap is created. In Conventional LASIK, an instrument known as a microkeratome, which is a hand held, high speed oscillating blade, is used to create the micro thin flap. Most of the complications in the LASIK procedure, have their origins in the use of this hand held blade instrument. Bladeless LASIK, on the other hand, uses a laser known as a femtosecond laser, to create the micro-thin flap. This is also called intralase lasik or i-lasik or femto-LASIK. In Bladeless LASIK, the entire process is automated, and is much safer, more precise and smoother overall for the patient.

Following are the Laser Eye Correction Risks. Please note that most of these outcomes are very rare, and in general, most laser eye correction patients do extremely well, and are off to work on th very next day with perfect outcomes.

a) Procedure may not be completed on the same day-generally with bladeless lasik or ReLEx smile, this is unlikely-and may need to be postponed by a few weeks.

b) Undercorrection or overcorrection, resulting in some number being left behind. Generally speaking, we offer a free enhancement procedure to take care of this. Our current enhancement rates are 1-2%. ReLEx smile enhancement rates are about 0.2%.

c) Post-operative infection. Extremely rare if proper care is taken

d) Excessive Corneal thinning. We will not treat patients whose corneas are already too thin. But in some cases, very rarely, excessive thinning can happen despite our best efforts.

e) Night Vision DIsturbances, like glare and haloes at night. Generally, with the best profiles like aspheric lasik or wavefront customized LASIK this is not likely.

f) Another Laser Eye Correction Risk is an eye infection. An eye infection can have disastrous consequences if not treated properly. This is why we advise antibiotic drops prior to the procedure, and after the procedure, and follow the strictest infection control protocols during surgery. Our infection rate is about 1 in 5000.

g) Flap complications-Only likely with LASIK and not with Bladeless LASIK or ReLEx smile.

How long does laser eye correction last?

Laser Eye correction works by corneal reshaping, and the shape change is a permanent one. Since the shape change is permanent, laser eye correction is also permanent. So, in general, laser eye correction last’s permanently, The refractive error (or number, in layman’s terms) should not come back following the procedure.

However, this statement should be viewed with some limitations. Laser eye correction may not last permanently if

a) The surgery has been done at a young age (i.e, less than 18 years for women and less than 21 for men)

b) Other pathologies develop in the eye, like a cataract or glaucoma.

c) Other systemic problems occur-for example, if there is pregnancy or diabetes. Refractive errors can also change as a reaction to some drugs. Sometimes contraceptive pills can also cause a change in refractive errors.

d) In a very few individuals, there is some change in power even without any laser eye correction. Such people should expect that laser eye correction will not last permanently in their case.

e) there is an abnormal healing reaction after laser vision correction-normally this will become evident  fairly quickly-around a month or so after laser eye correction. In such a situation, New Vision Laser Centers will perform an enhancement procedure free of charge.

Also, one must remember that laser vision correction is for distance vision correction. Don’t imagine that it will work for age related near vision problems which appear after the age of 40-42. Such people should undergo presbyopic lasik or laser blended vision.

Dr. Rupal Shah was invited as a speaker at the Carl Zeiss European-Middle East-Africa User Meeting on refractive lasers held on June 1, 2013 at Majorca, Spain. She spoke on ReLEx smile-Learnings from Clinical and Scientific Studies. She gave an overview of all the experience gained from ReLEx smile studies through the last 5 years at New Vision Laser Centers-Centre for Sight. Her talk highlighted the advantages of ReLEx smile, including its beneficial effects on Stability, Dry Eyes and Corneal Strength. Dr. Shah was the only Indian invited to this primarily European-Middle East Meeting.

President Obama felicitates Indian Pioneer of LASIK surgery
